The barrier to a student getting cleared is rarely the exam. It is a parent taking time off, transport, and a clinic appointment three weeks out. Coming to campus removes all three, and it is why on-site days reach students who would otherwise miss a season or play uncleared.

The history form is the part that needs a parent

The cardiac history - fainting during exercise, chest pain on exertion, a relative who died suddenly and young - is the highest yield part of the whole evaluation, and a 15-year-old often cannot answer it. Forms need to go home and come back completed. A form filled in by the student in the queue defeats the purpose of the day.

What happens when a student is not cleared

They are told privately, not in front of teammates, and the athletic department receives the clearance status without the medical detail. The student and family get the detail and a clear next step. Most students referred for further evaluation are eventually cleared to play - being held back on the day is a pause, not a verdict, and saying that plainly prevents a lot of distress.

Records the athletic department can use

A roster-level clearance list showing cleared, cleared with conditions, and pending, so a coach can see at a glance who may take the field. Medical detail stays in the confidential file. Coaches do not need diagnoses and should not have them.
